Affordable Dental Care Without Traditional Insurance

• This plan cannot be combined with any other insurance or discount offers.
• This plan may be used as coverage in the event yearly benefits of traditional insurance have been maxed out and there is a need for coverage before the next benefit year renews at the full cost.
• Eaglin Dental Savings Plan lasts from the date of purchase for one annual year and must be renewed on the date as its original purchase to continue.
• If a patient obtains insurance after purchasing this plan, the patient must choose one or the other to use at Eaglin Dental Group.
• Patient is responsible to utilize the cleanings prior to the plan’s expiration date.

If cleanings have not been used in the time frame of one year from purchase, this plan does not allow that cleanings roll over after the date of expiration. This plan can only be used at the same location it was purchased at ONLY. If this plan is used at multiple locations this will result in being charged for the full price of the cleaning at the location outside of where the plan was originally purchased and possible early termination of the savings plan without refund or patient may not be given an option to renew.
